How care is delivered
Conservative treatment comes first
Evaluation begins with the least invasive effective treatment, reserving surgery for cases where non-operative options are no longer sufficient. Surgery is generally reserved for symptoms that do not improve, or for conditions involving progressive neurological problems, significant instability, infection, trauma, or another urgent concern.
A non-operative evaluation reviews symptoms, medical history, previous treatment, medications, occupation, activity level, and treatment goals. The physical examination assesses strength, sensation, reflexes, balance and walking, spinal movement, and areas of tenderness. Imaging is reviewed and additional testing may be recommended.
Non-surgical treatments offered or coordinated include:
- Lifestyle and activity modification
- Physical therapy
- Occupational therapy
- Pain-management coordination
- Continued monitoring
Second opinions
The practice provides independent second opinions for patients who have been advised to undergo cervical, thoracic, or lumbar surgery, who have received conflicting diagnoses, or who want clarification before deciding.
To request one, call either office or submit the appointment request form. Patients are asked to provide MRI, CT, and X-ray images, radiology reports, operative reports, implant information, physical therapy records, injection records, medication lists, and prior specialist notes. Providing the actual imaging files, not only the written reports, is important to the review.
Surgical technology used
The practice uses the following systems during surgery. Each supports smaller access, more accurate placement, or greater protection of neural structures. Benefits depend on the individual diagnosis, procedure, and health status.
| Technology | Purpose |
|---|---|
| Intraoperative neuromonitoring (IONM) | Continuously assesses nerve function during surgery to help prevent neurological injury. |
| Intraoperative navigation (Q Guidance) | Provides real-time three-dimensional guidance for trajectory planning and screw positioning. |
| Ultrasonic bone-cutting device (Sonopet) | Uses high-frequency vibration for precise bone sculpting while protecting surrounding soft tissue and nerves. |
| Robotic assistance (Co-Pilot) | Supports precision in decompression and screw placement and allows smaller incisions. |
| Surgical microscope | Provides high-powered magnification and illumination of neural structures. |
| Augmented reality | Used for surgical planning and visualization in cases with complex anatomy. |
Dr. Ortega has also participated in professional education content on Stryker MAKO spinal robotics and on the Kyocera Medical Tesera-K system for anterior lumbar interbody fusion.
